Chye Clinic半月板撕裂会有什么感觉
半月板是膝关节的避震垫。它撕裂时，症状往往很具体，而不是笼统的酸痛。
- 疼痛集中在内侧或外侧关节线，而不是整个膝盖
- 肿胀在数小时之内出现，而不是当下立刻肿起
- 有卡住、喀响，或觉得膝盖「靠不住」
- 蹲下、扭转或下车时疼痛
- 卡锁：膝盖卡住、无法伸直
膝盖卡住无法伸直、反复无力打软，或受伤当下立即肿起，都应尽快评估，不要拖。
两种半月板撕裂
分辨自己属于哪一种，正是决定治疗方式的关键。这是本页最有用的一项区分。
外伤性
- 发生在原本健康的半月板上，因瞬间受力所致
- 通常是脚掌固定、身体扭转
- 运动场上的伤，较常见于年轻的膝盖
- 撕裂很大，或造成膝盖卡锁，才是真正会考虑手术的情况
退化性
- 发生在已随年龄变得脆弱的半月板上
- 往往没有明显外伤：只是蹲下，就觉得「有东西滑掉了」
- 通常伴随早期膝关节退化，也当作退化的一部分来处理
- 对大多数人而言，有系统的运动训练效果与关节镜手术相当
MRI 回答不了的问题
理学检查会按压关节线，并以特定方式让半月板承受负荷，往往就足以做出诊断。MRI 能直接看见撕裂，会在结果足以改变处理方式时使用，而不是例行安排。
真正的问题很少是「有没有撕裂」。在中年之后的膝盖里，撕裂相当常见，而且经常没有症状。要问的是：这一处撕裂，能不能解释这些症状。这个问题靠检查人来回答，不是靠扫描。
如何治疗
多数退化性撕裂无须手术即可处理，而支持这一点的证据逐年增加。
初期调整活动方式
之后逐步加载，而不是长期静养；长期静养会让保护关节的肌肉变弱。
物理治疗
恢复股四头肌肌力与膝关节的控制。这是治疗本身，不是治疗之后才做的事。
视需要止痛
消炎止痛药，用来让加载做得下去，而不是取代加载。
注射治疗
若合并膝关节退化时使用，而年长的膝盖多半合并退化。
有需要时转介手术评估
膝盖卡锁、较大的外伤性撕裂，或症状始终不缓解时。

The clinic is clean, the staff are professional and welcoming, incredibly efficient. The clinic interior design also calming and made my visits less anxious. The doctors here (Dr. Chan & Dr. Kon - sorry if misspelled) are very attentive, explain briefly to me what the do's and don'ts on after care treatments and for future preventions. I also just learned that this clinic is specialized in skin problems and i refer my dry and ictchy skin problems on my fingers. I've been put some plasters to hide the crack and dry skin for months. The level of care provided was excellent, and the treatment received was highly effective. Healthcare environments can sometimes feel stressful, but this clinic has a wonderful, relaxing atmosphere. - 半月板撕裂会自己好吗？
- 外三分之一有血液供应，有机会愈合。内侧部分大多无法愈合，因此治疗的目标放在功能与症状，而不是把撕裂「补起来」。
- 一定要做 MRI 吗？

- 对年轻人的大型外伤性撕裂与卡锁的膝盖，效果明确。对合并退化的退化性撕裂，多项研究显示手术不比有系统的运动训练好，所以那一类先不开刀。
