When screening should start, by age

The useful question is not whether to have a check-up. It is which tests earn their place at the age you are now, because a panel that is right at 55 is mostly wasted at 25.

  • You have reached an age where something new is worth starting
  • A parent or sibling has diabetes, heart disease or a cancer that runs early
  • You have never had bloods done, and cannot say what your numbers are
  • Your last screening was long enough ago that you cannot remember it
  • You are being screened for work and want to know what it does not cover

Age sets the starting point, not the whole answer. A family history of early heart disease or bowel cancer moves the start date forward, sometimes by a decade.

What a health screening covers at each age

  1. 20sBlood pressure, weight and a baseline set of bloods. Cervical screening once you are sexually active, and an STD screen if that is relevant. Thalassaemia carrier status is worth knowing before you plan a family, not after.
  2. 30sThe same, plus blood sugar and cholesterol taken seriously rather than as a formality, particularly with a family history or a rising waistline. Cervical screening continues on its interval.
  3. 40sWhere screening starts to earn its keep. Glucose and lipids every time, liver and kidney function, and an abdominal ultrasound if there is metabolic risk. Breast screening becomes a conversation to have rather than one to defer.
  4. 50sBowel screening enters the picture, and it is the test people most often skip. Breast screening settles into a regular interval. For men, whether to test the prostate is a decision to make with a doctor rather than a box to tick.
  5. 60s and beyondEverything above continues, and vaccination status starts to matter as much as the bloods: influenza yearly, pneumococcal once the age threshold is reached. Vision, hearing and bone health join the list.

How an age-based screening visit works

  1. The consultation comes first

    Your age, your family history and what you have already had done. This is what decides the panel, which is why it is not sold in advance.

  2. Bloods on the day

    Taken at the clinic. Come fasted if glucose and lipids are on the list, and ask when you book rather than guessing.

  3. Anything the age bracket adds

    An abdominal ultrasound, cervical screening, or a referral onward where the test is not one a clinic does. You are told which of these apply before anything is run.

  4. Results, explained

    Gone through by the doctor who ordered them. A number outside the reference range is not a diagnosis, and it is the explanation that decides what happens next.

  5. A date for the next one

    Screening is an interval, not an event. You leave knowing when the next one is due rather than working it out years later.

What age should I start health screening?
Blood pressure and a baseline set of bloods are worth having from your twenties. The tests that most change outcomes, glucose, lipids and bowel screening, start in the forties and fifties. A family history of early heart disease or bowel cancer moves that forward, sometimes by ten years.
How often should I repeat it?
For most people in good health, every one to two years is enough, and each test has its own interval. You leave the visit knowing when the next one is due rather than working it out later.
Do I need to fast?
If glucose and lipids are on the list, yes, usually eight to ten hours. Ask when you book, because it depends on what is being run and it is a wasted trip if you get it wrong.
Is a screening the same as a pre-employment medical?
No. A pre-employment or FOMEMA examination is done to answer an employer's question, and it is not designed to find the things a screening looks for. People are often surprised by how little it covers.
Can I just pick a package myself?
You can, but the panel is better decided after a consultation than before one. Your age and your family history are what make a test worth running, and neither is on a price list.
