Who premarital screening is for
It is a screening done once, before a marriage, to answer two separate questions: whether either of you carries an infection worth treating now, and whether the two of you together carry a risk you would want to know about before having children.
- You are getting married and one or both of you needs the screening for the registration
- You are planning children and want carrier status settled beforehand
- Either of you has a family history of thalassaemia, which is common enough in Malaysia to be worth ruling out
- Neither of you has had a full infection screen, and you would rather start from a known position
- One of you has had a previous partner and would prefer to check rather than assume
If you need the HIV test on the state religious department's own form, say so before you come. That form has to be issued by a clinic on their approved list, and it is worth confirming rather than finding out on the day.
What a premarital screening covers
Two different jobs in one visit. The infection tests are about now; the carrier tests are about children you have not had yet.
- HIV
- A 4th-generation test. This is the one the state religious department requires of Muslim couples before marriage, and it is worth having regardless of whether it is required of you.
- Hepatitis B
- Surface antigen, to find a carrier. It matters because hepatitis B passes between partners and from mother to baby, and because a partner who is not immune can be vaccinated.
- Syphilis
- Treatable completely when it is found, and capable of causing serious harm in a pregnancy when it is not.
- Thalassaemia carrier status
- A full blood count with red cell indices, and further testing where those point to it. Two carriers have a one in four chance with each pregnancy of a child with the severe form, which is precisely the thing this screening exists to find.
- Blood group and Rh
- Worth knowing before a pregnancy rather than during one.
- Rubella immunity, for the woman
- Rubella in early pregnancy causes serious birth defects. If she is not immune, vaccination before conceiving is straightforward, and afterwards it is not an option.
How a premarital screening visit works
Come together, if you can
It is one visit and one blood draw each. Coming separately is fine, but the carrier results only mean something read side by side.
A short consultation
Family history on both sides, vaccination history, and anything either of you wants tested that is not on the standard list.
Bloods
One draw each. No fasting is needed for this panel.
Results, explained to both of you
Each result belongs to the person it came from and is given to them. What the two sets mean together, particularly on carrier status, is explained to you as a couple.
What follows, if anything does
Vaccination where someone is not immune, treatment where something is found, and a referral for genetic counselling if both of you turn out to be carriers.
What premarital screening cannot settle
A carrier result is not an illness. Most thalassaemia carriers are entirely well and will stay that way, and the finding matters for the children rather than for the person carrying it. Being told you are a carrier is not being told you are unwell.
The infection tests are also a snapshot of the day they were taken. HIV in particular has a window period after an exposure during which a test can be negative and the infection still present, so if there has been a recent exposure, say so, because the timing changes what the result is worth.
And a screening does not decide anything for you. It gives two people the same set of facts before they make plans, which is the whole of what it is for.

The window period explained properly, and what a negative at four weeks does and does not mean.
Vaccination
Hepatitis B for a partner who is not immune, and rubella before conceiving rather than after.
Health screening by age
If this is the first time either of you has had bloods done, it is worth knowing what else your age calls for.

Common questions
- Is premarital screening compulsory in Malaysia?
- The HIV test is required of Muslim couples before marriage in every state. The rest of the panel is not compulsory for anyone, and it is the part that most often changes what a couple does next.
- Can we both be done in the same visit?
- Yes, and it is better that way. It is one blood draw each, and the carrier results only mean something when they are read side by side.
- Do we need to fast?
- No. Nothing on the standard premarital panel needs fasting.
- What happens if we are both thalassaemia carriers?
- It means a one in four chance with each pregnancy of a child with the severe form. It does not mean you cannot have children, and it is the point at which genetic counselling becomes worth having, so we refer you.
