Genital herpes symptoms people notice first
Most people do not arrive saying they think they have herpes. They arrive with a symptom, and often one they have been hoping would clear up on its own.
- Small fluid-filled blisters that burst and leave raw ulcers
- A painful ulcer near the vagina, vulva, penis, scrotum, groin, buttocks or anus
- Burning pain on urination, when a sore sits near the urinary opening

Come in while the sore is still there if you can. A swab from an active blister gives the clearest answer, and antivirals work best started in the first day or two.
Why an ulcer needs looking at rather than guessing
Genital ulcers are not all herpes. Syphilis, bacterial infection, fungal infection and simple skin irritation all produce sores that look similar, and the treatments are entirely different.
Herpes itself is also frequently mistaken for a pimple, heat rash or an allergic reaction. That is part of why it keeps spreading: people with mild or unrecognised symptoms still transmit it.
How genital herpes is diagnosed
An examination, and then whichever tests the sore itself makes appropriate.
- Examination
- What the lesion looks like, where it is, and how it has behaved so far narrows the list before any test is run.
- HSV swab or PCR
- Taken from an active blister. This is the most reliable test, and the reason to come in while a sore is present rather than after it has healed.
- HSV-1 and HSV-2 antibodies
- A blood test, useful where there is no active lesion to swab or where past exposure is the question.
- A wider STI screen
- An ulcer warrants ruling out syphilis and HIV, which produce sores of their own and need entirely different treatment.
What treatment changes, and what it does not
Herpes cannot be cleared from the body. For most people, the first column is the part that actually matters.
Antivirals do
- Shorten an outbreak, especially started in the first day or two
- Reduce the pain while it lasts
- Reduce how often outbreaks come back, taken as daily suppressive therapy
- Lower the risk of passing the virus to a partner
Antivirals do not
- Clear the virus from the body; nothing currently does
- Prevent every future outbreak
- Remove the need to tell a partner
- Treat the other things that cause genital ulcers

The clinic is clean, the staff are professional and welcoming, incredibly efficient. The clinic interior design also calming and made my visits less anxious. The doctors here (Dr. Chan & Dr. Kon - sorry if misspelled) are very attentive, explain briefly to me what the do's and don'ts on after care treatments and for future preventions.
