PrEP is worth asking about if any of these apply
PrEP is for people who are HIV-negative and expect to stay at some risk. It is not an emergency measure, and it is not only for one group.
- You have a partner living with HIV whose virus is not suppressed
- You have had condomless sex, or more than one partner
- Men who have sex with men, at ongoing risk of exposure
- You have recently been diagnosed with another STI
- You have taken PEP and want continuing protection
If a possible exposure has already happened in the last 72 hours, PEP is the conversation and not PrEP. Say so when you message and we will treat it as urgent.
Daily or on-demand
Two regimens, and which one fits is a decision to make with the doctor rather than from a website.
- Daily
- One tablet every day. This is what most people are on, it covers exposures that were not planned, and it is the simpler habit to keep.
- On-demand, 2-1-1
- Two tablets before, one at 24 hours, one at 48. It suits people whose exposures are infrequent and predictable, and it is not appropriate for everyone.
- Generic or branded
- Both are stocked. Tenofovir/emtricitabine is the same molecule either way; the difference is the price, and generics start at RM 109 a month.
- Collected or delivered
- You do not have to come in for every refill. Where it suits you better, the medication can be delivered.
How it works at the clinic
Consultation
Your risk, how the medication works, and its side effects. PrEP is prescription-only in Malaysia, and this is the appointment that produces the prescription.
Baseline tests
HIV status, kidney function, hepatitis B, and an STI screen. Starting PrEP while already infected risks drug resistance, which is why the HIV test is not optional.
Starting the course
The regimen is chosen with you, along with a plan for how you will actually take it. Protection drops sharply if doses are missed.
Review every three months
HIV test, kidney function and STI screening. This is ongoing care rather than a repeat prescription handed over a counter.

Common questions
- Do I need a prescription for PrEP in Malaysia?
- Yes. PrEP is prescription-only, and a prescription follows a consultation and a baseline HIV test. Starting PrEP while already infected risks drug resistance.
- Is PrEP safe?
- Tenofovir/emtricitabine is approved by the WHO and the US FDA and has a long safety record when taken as directed. Most people have no side effects; some have mild nausea, headache or stomach discomfort in the first few weeks, which usually settles.
- How often do I need testing while on PrEP?
- Every three months: an HIV test, kidney function, and STI screening.
- Can I drink alcohol on PrEP?
- Alcohol does not interact with the medication. Heavy drinking matters indirectly, because missed doses reduce protection.
- Can I stop taking PrEP?
- Yes, if your risk changes, but discuss it first, and continue for seven days after the last possible exposure so you are covered through the washout period.
- Can I take PrEP if I have kidney or liver problems?
- Tell the doctor before starting. PrEP is cleared by the kidneys, so a baseline kidney function test and regular monitoring are required, and a history of kidney disease changes the assessment.
- Do I still need condoms on PrEP?
- Yes, if you want protection from other sexually transmitted infections. PrEP covers HIV only.
