Chlamydia symptoms, and when to test anyway

Chlamydia is the most common bacterial STI and the quietest. Waiting for a symptom is not a way of finding it, because for most people there is never going to be one.

  • Unusual discharge, burning on urination, or pain during sex
  • Lower abdominal pain, or testicular pain and swelling
  • Rectal pain, discharge or bleeding after anal sex
  • A partner diagnosed with chlamydia or any other STI
  • A new partner, or more than one, since you last tested
  • No symptoms at all, and you are sexually active

Around 70% of women and many men have nothing to notice. That is the entire case for testing rather than waiting.

Where it lives, and how each site is tested

Chlamydia trachomatis is passed on through vaginal, anal and oral sex, and from mother to baby at birth. For most people a urine sample is all that is needed.

Genital tract
The usual site. In women: unusual discharge, burning on urination, pain during sex, lower abdominal pain. In men: discharge, burning, or testicular pain and swelling. A urine sample covers it.
Rectum
Pain, discharge or bleeding, though usually nothing at all. Tested with a rectal swab where the exposure points there.
Throat
Almost always symptomless, and found only by swabbing for it after oral sex.
Eyes
Conjunctivitis, from contact with infected fluids. Tested with an eye swab.

Chlamydia treatment, and what has to happen around it

  1. Antibiotics

    Either a single dose of azithromycin or a week of doxycycline. Finish the course even after symptoms settle; stopping early risks the infection returning.

  2. No sex for seven days

    After a single-dose treatment, or until a longer course is finished.

  3. Partners tested and treated

    Whether or not they have symptoms. Otherwise the infection passes straight back once your treatment finishes.

  4. A retest, a few months on

    Often sensible, because reinfection from an untreated partner is the most common reason chlamydia comes back.

Why an infection with no symptoms is worth finding

In women, chlamydia can ascend to cause pelvic inflammatory disease: chronic pelvic pain, fever, and scarring of the fallopian tubes. That scarring is what causes tubal infertility, and it raises the risk of ectopic pregnancy, a fertilised egg implanting in a damaged tube, which is a medical emergency.

In men it can cause epididymitis: pain and swelling in the testicle, sometimes with fever, and in rare cases lasting damage.

None of this is common in people who get treated. All of it starts with an infection that had no symptoms.
