Fatty liver causes almost nothing until it has been there a long time. It is found because someone looked, not because someone felt it.

  • Type 2 diabetes, prediabetes or insulin resistance
  • Overweight, and particularly weight carried around the middle
  • High cholesterol or high triglycerides
  • Regular alcohol, at any level you would describe as regular
  • Persistent tiredness or a dull ache under the right ribs

The first line is the commonest way this is found. A mildly abnormal liver result on an unrelated blood test is worth following up rather than repeating and forgetting.

What fatty liver actually is

Fatty liver disease, or hepatic steatosis, is fat accumulating inside liver cells. It splits into two: alcoholic fatty liver disease, linked to how much you drink, and non-alcoholic fatty liver disease, which happens in people who drink little or nothing and is driven by weight, blood sugar and blood lipids.

It affects roughly a quarter of adults worldwide. Left alone, fat leads to inflammation, inflammation leads to fibrosis, and fibrosis can end in cirrhosis. The important part is the other direction: at the fatty stage, before scarring, this is one of the more reversible chronic conditions there is.

How it is found and assessed

  1. Blood tests

    Liver enzymes first, then the things that drive it: blood sugar, HbA1c, cholesterol and triglycerides. A liver result alone does not tell you why.

  2. Abdominal ultrasound

    The usual way fat in the liver is actually seen. It is painless, takes minutes, and is included in the clinic's larger screening packages.

  3. Ruling out the other causes

    Hepatitis B and C, alcohol intake taken honestly, and medications that affect the liver. Fatty liver is common, which makes it easy to assume and worth confirming.

  4. A plan, and a date to recheck

    Enzymes and weight are re-measured rather than assumed to have improved. This is a condition managed by numbers over months.

What changes it, and what does not

Worth being straight about, because there is more marketing in this area than there is evidence.

What the evidence supports

  • Losing 7 to 10% of body weight, which is the single most effective treatment
  • Cutting alcohol, whichever type of fatty liver you have
  • Treating the diabetes, cholesterol and blood pressure alongside it
  • Regular exercise, independently of weight lost
  • Rechecking the bloods, so you know whether any of it worked

What no drip will do

  • Reverse fatty liver on its own
  • Substitute for weight loss or for cutting alcohol
  • Undo established fibrosis or cirrhosis
  • Justify skipping the blood tests that show whether it is working

Where the glutathione drip fits

The clinic offers a liver detox infusion with glutathione, and it is worth saying plainly what it is and is not. Glutathione is the body's principal antioxidant and the liver depends on it heavily; the case for supplementing it is a reasonable one, and early work is promising.

What does not exist is trial evidence that an infusion reverses fatty liver. It is offered as something taken alongside the weight, alcohol and metabolic work above, not instead of it, and any clinic presenting it as the treatment is selling you something the evidence does not support.

Does fatty liver cause symptoms?
Usually none until it is advanced. Some people notice persistent tiredness or a dull ache under the right ribs. It is normally found on a blood test or an ultrasound done for another reason.
Is fatty liver reversible?
At the fatty stage, yes, and that is the point of finding it early. Once fibrosis or cirrhosis has developed, the damage already done does not reverse.
I do not drink. Can I still have it?
Yes. Non-alcoholic fatty liver disease is the commoner form and is driven by weight, blood sugar and blood lipids rather than alcohol.
Will a glutathione drip fix it?
No, and we would rather say so. The liver depends on glutathione and supplementing it is a reasonable idea, but there is no trial evidence that an infusion reverses fatty liver. It sits alongside weight and alcohol changes, not instead of them.
